Use o-rings
I trashed a couple bushings before figuring out the solution. Get some small rubber o-rings at the hardware store (a couple bucks) and slip them on the part where the bushing slides on. The size should be smaller in diameter such that it stretches over the part. Lube up everything with triflow and you will hopefully be good to go. Prior to doing this, the tool pushed too hard on the bushing towards the end of insertion and caused the edges to mushroom out and roughen, causing the retainers to not be able to insert.
